  1. I, the undersigned, authorize Crate's to proceed with the above work, and charge the credit card listed above upon completion of the work.
  2. I also authorize the service department to operate the engine(s) and vessel for the purpose of testing and/or inspection.
  3. All vessels must have valid insurance with Crate's listed as additional insured.
  4. Unless otherwise stated, all prices are for labour only — parts are extra.
  5. All work orders are due and payable immediately upon completion of the specified work. Crate's will charge the credit card on file upon completion.
  6. Customer accounts must be in good standing prior to the commencement of any work.
  7. Interest shall be charged at 2% per month on all outstanding invoices.
  8. It is understood and agreed by the customer, his/her agents, heirs and executors that Crate Marine will have legal rights under the Storage and Liens Act for all work performed on the vessel.
  9. The vessel must be insured by the owner for the duration of the storage period.
  10. Crate Marine Belleville respects our customers’ privacy. All personal information gathered during business transactions is used solely for internal billing purposes, is protected with the utmost care, and will remain within our company files.
